Consult Center (OCC).
Your first step in joining one of
our programs is to fill
out the application form and either
fax it back to us at 617 636-2386 or
mail it to 750 Washington Street, NEMC
#900, Boston, MA 02111. Attention Kathy.
We are experts in the treatment of
obesity. Our goal is to help you achieve
and maintain a life-long healthy weight.
Our treatment team includes behavior
health experts, physicians (internists
and surgeons), a Physician Assistant
and Registered Dietitians.
We offer both surgical programs
and physician-supervised/behavioral weight
loss programs:
The physician-supervised / behavioral
weight loss program is personalized
to your needs and has many options
including dietary and lifestyle change
groups, formula diets (Optifast)
and the use of medications. Most of
our programs are offered in both individual
and support-group formats.
The surgical program offers
either the Gastric Bypass or Lap-Band
surgery and includes a full behavioral,
medical and nutritional pre-op and
post-op support structure.
For patients choosing the surgical
option, there is a mandatory $400 program
fee payable at the time of your first
visit.
Even if you are certain of the program
you wish to join, please come to one
of our monthly 'New
Patient Orientation' sessions.
Information about our various weight
loss programs will be presented by
one of our staff. There is no charge
for this meeting and no appointment
is necessary. Family and support members
are welcome. Following this meeting,
we suggest that you stay and listen
to our patients who have had the surgery
discuss their experiences.
Please review the enclosed materials
accompanying this letter. Depending
on your insurance, you may be responsible
for co-pays and/or fees to cover clinic
visits and group sessions.
Please be patient. We will get back
to you as soon as we can. If you are
accepted into the screening phase of
our program, you will be called to
schedule an initial appointment.
